The Medical form consists of four pages and all four are required. We understand some Doctors like to use their own form, this is OK but it still has to be accompanied by ours filled out by a parent and signed. Note there is a location on the form to indicate who has your permission to pick up your child. It is our policy to only release campers to the people who are listed on the form. Everyone adult and child staying over night must have a medical form. If you arrive at camp without one you will be asked to leave.
